Job Description 

Position Title:            Supportive Housing Case Manager

Reports To:                Housing Director

Job Classification:   Full-Time, Non-Exempt

Supervises:               N/A

 

Position Purpose: In support of the AIM Center’s mission, to create hope, community, and social change to enrich and celebrate the lives of people living with mental illness. This position is responsible for promoting community integration and providing on-site support for housing residents to meet the obligations of their lease. Using a person-centered, culturally responsive, and evidence-based trauma-informed approach, the supportive housing case manager will promote independence, community belonging, social capital, and well-being. This position supports residents in accessing community resources and developing skills to re-integrate into the community through a supportive, professional relationship.

                                    Essential Duties and Responsibilities 

Case Management Duties 

·      Support residents through assessment, planning, implementation, coordination, monitoring, and evaluation.

·      Collaborate with AIM Center program staff and housing residents to develop a service plan addressing housing needs for applicants, with evaluation and updating every six months.

·      Monitor progress or lack of progress toward housing goals set forth in individual service plans and assist in revisions as necessary.

·      Complete all required documentation that accurately and fully describes the resident’s progress toward their goals and support activities.

·       Notify AIM Center clubhouse support staff, case managers, and other identified support persons of resident issues as appropriate.

·       Serve as liaison between residents and housing management regarding complaints and grievances.

Promoting Stable Housing and Independent Living

·  Teaching appropriate social behavior, and other skills to help promote a person’s independence while in the community.

·    Assist people in advocating for themselves and learning more about

their rights.

·   Determine the need for and provide or arrange for life skills

instruction either on or off-site.

·    Assist in monthly CSH Property Inspections.

·    Assist in scheduling and facilitating resident screening meetings.

·    Assist with move-in processes.

·    Plan and attend monthly residential socials.

·    Create and distribute monthly calendar working side by side with a resident member.

·    Distribute/inform residents of specific AIM Center unit opportunities including classes, socials, and wellness & recovery programs.

·    Serve as liaison between residents and housing management regarding complaints and grievances.

Relationship and Community Building 

·   Develop and maintain a professional and considerate rapport with residents receiving support while promoting concepts of self-determination and dignity of risk.

·   Help residents explore different areas of town and local activities, maintain or build community relationships, depending upon the person’s goals and wishes.

·   Promote residents’ participation in educational, employment, support, recreational, and enrichment opportunities available at the AIM Center and in the community, including Clubhouse socials.

·   Provide opportunities for community engagement, and help residents explore their community interests to increase/maintain their community participation.

·    Plan and attend monthly resident meetings.

·    Encourage and facilitate the organization of peer support groups, resident activities, programs, and speakers.

·    Build positive relationships within the community including community members, law enforcement, housing, and social service agencies.

·     Attend neighborhood and community meetings as appropriate.

Other  

·     Provide transportation for housing residents to community events and activities, including those hosted at the AIM Center.

·      Maintain necessary competence to complete job duties as described including necessary training and certifications as assigned.

·    Assist the Housing Director in housing and administrative functions as needed.

·    Assist in other job duties as assigned in the Housing Resource Center.

·    Participates in Clubhouse social and holiday staff rotation coverage.

                                                                     Required

Education/Training/Experience:

·    Bachelor’s degree in social services or a directly related field or equivalent experience.

·    Ability to work independently and produce quality work under deadlines.

·    Excellent written and verbal communication skills.

·    Ability to set and maintain positive and professional boundaries with residents.

·   Intermediate computer literacy and familiarity with applications such as databases and spreadsheets, word processing, email, and the internet.

                                                                      Preferred 

·  Experience in the mental health field preferred.

·  Bilingual in English and Spanish or other languages.

·  Knowledge of Hamilton County resources.

                                        Mandatory Qualifications/Skills/Traits 

·   Pass a pre-employment background check.

·   Valid Tennessee driver’s license. Ability to obtain an F-Endorsement within 14 days (Tennessee Residents only), 3-year clean MVR, NO DUI/DWI.

·    Reliable transportation and automobile liability insurance.

·    First Aid and CPR certification or willingness to obtain First Aid and CPR certification.

·    Ability to read, write, and understand verbal and written instructions.

·    Successfully complete all training, including online Relias Learning courses.
