CHOICES Case Manager
The Southeast Tennessee Area Agency on Aging and Disability is seeking a Choices Case Manager.
Responsibilities include traveling within a 10-county region, completing assessments for nursing home level of care and assisting with Medicaid applications. Knowledge of Pre-Admission Evaluation and Medicaid a plus.
The ideal candidate will have excellent communication skills (both written and verbal), be able to maintain confidential information, able to work independently and with a team. Candidates must be able to establish and maintain positive and effective working relationships with internal/external contacts and partners.
Must have a current Tennessee license MSW or BSW and have at least two years of experience, preferably in geriatrics or home health care.
