The Southeast Tennessee Area Agency on Aging and Disability is seeking a Case Manager.

Responsibilities include conducting assessments to determine eligibility for home and community-based services such as home delivered meals, homemaker, personal care, and respite.

The ideal candidate will have excellent communication skills (both written and verbal), be able to maintain confidential information, able to work independently and with a team.  Candidates must be able to establish and maintain positive and effective working relationships with internal/external contacts and partners.

Must have an Associate degree and two (2) years of experience in social work or a related field.  A Bachelor’s Degree in social work field is preferred.
