Program Summary: Northside Neighborhood House envisions a thriving community working together to facilitate cross-generational stability, empowerment, and elimination of barriers for all of our neighbors. Through a collaborative approach, NNH provides basic needs support in addition to access/educational opportunities for neighbors to achieve short and long-term goals that will lead to economic mobility.
Position Summary: The Stability Case Manager provides comprehensive, trauma-informed stability services to individuals and families, serving school-based referrals alongside those from the broader community. By fostering intentional relationships with both neighbors and community partners, this role empowers neighbors to overcome barriers, access vital resources, and achieve their goals.
Essential Functions & Responsibilities:
- Schedule: Typical schedule will be Monday-Friday from 8:30am-4:30pm. Regular, punctual attendance is expected of all employees. Schedule may vary as needed to fully support programming.
- Travel: Local travel on a regular basis throughout Hamilton County required. Reliable transportation and valid driver license is required to perform all functions of this role effectively.
- Equipment/Technology: High level of computer competency required, including proficiency with Google Workspace (specifically for email, calendar, and spreadsheets). Must demonstrate the ability to quickly master and maintain precise records within the neighbor database (Sumac).
- Physical Requirements: Ability to sit, stand, bend, reach, and lift up to 40lbs. Manual dexterity sufficient to operate standard office machines required.
Essential Functions & Responsibilities:
Case Management (80%):
- Partner with the Scheduling Specialist to support intake workflows, ensuring school and community referrals are processed seamlessly.
- Engage with neighbors to assess their needs, provide practical assistance, and empower them to achieve self-sufficiency.
- Develop personalized short- and long-term Stability Action Plans, addressing root causes of challenges.
- Assist neighbors with financial literacy, including creating household spending plans and SMART goal-setting.
- Provide work readiness support, such as resume building and job search guidance.
- Distribute resources, such as food, personal care items, and financial assistance, while ensuring compliance with organizational policies.
- Offer referrals to appropriate external services when NNH cannot meet a neighbor’s specific needs.
- Maintain precise records within Sumac, our neighbor database, to ensure all service delivery and community impact data is captured accurately.
Community Engagement (20%):
- Build and maintain partnerships with local businesses, schools, and community organizations to enhance resources available to neighbors and the Stability Team.
- Facilitate and support community education workshops, monthly CEAP meetings, and bi-monthly Community Advisory Board (CAB) sessions to address and advocate for neighbors’ needs.
- Represent NNH at community functions to advocate for stability programming and expand awareness of available services.
- Assess community strengths and needs, bringing together stakeholders to develop solutions to address challenges.
- Recruit community members to participate in NNH events and programming.
- Attend training and professional development opportunities to stay informed on social service trends and best practices.
This position offers cross-functional support for stability operations, with a strong focus on team-wide collaboration. Responsibilities include providing essential coverage for the Scheduling Specialist (front desk, phones, and referrals) during leave, as well as assisting with community workshops to ensure our operations run smoothly. This role may perform additional mission-critical duties as assigned by the Chief Stability Officer or the CEO.
Typical Work Environment: While primarily office based, this role is also community-facing and requires travel to sites throughout Hamilton County. Work location & environment may vary as needed to fully support programming.
Qualifications:
Education/Experience:
- Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or a related field required; Master’s degree (MSW) preferred.
- Minimum of 2–3 years of experience in case management, social services, or support programs.
- Proven experience working with individuals or families through a trauma-informed lens.
- Demonstrated openness to receiving and integrating feedback.
- History of working productively within a team to reach collective goals.
- Familiarity with Hamilton County resources and local non-profit networks (Highly Preferred).
- Experience in community organizing or program development preferred.
Skills/Abilities:
- Ability to build rapport and foster “intentional relationships” with neighbors, coworkers and community partners.
- Sensitivity to the diverse socio-economic and cultural backgrounds of residents in Hamilton County.
- Ability to lead community education workshops, Advisory Board sessions, and CEAP meetings while professionally representing and promoting the NNH mission at all community functions and partner meetings.
- A proactive mindset for spotting community barriers and a desire to work with others to find creative solutions.
- Highly organized, dependable, and punctual; able to manage a high-volume caseload with attention to detail.
- Ability to guide neighbors through household spending plans, SMART goal-setting, resume building, and job search strategies.
- Exceptional listening skills to assess complex needs and develop actionable Stability Action Plans.
- Deep commitment to confidentiality, discretion, and safeguarding neighbor privacy within a social service context.
- Ability to remain calm, balanced, and tactful during high-stress situations or when supporting neighbors walking through high-stress situations.
- Willingness to pivot strategies based on community needs.
